Coordinated Care Advising
Mission
The Coordinated Care Advising model creates a supportive environment by providing proactive, holistic and individualized care for students that encourages and empowers their academic and personal success.
Coordinated Care Advising is an advising approach that supports and develops first-year and sophomore students by addressing academic, student life and wellness needs to foster their success at SDSU and beyond.
Coordinated Care Advisors* are dedicated staff members who are responsible for the advising and coaching of their first and second year student caseload to assist them in building the confidence and skills needed to thrive throughout their collegiate journey.
Coordinated Care Outreach is proactive, curated communication that supports the retention of our diverse student populations to increase their awareness and engagement with academic support and community opportunities.
Student Learning Outcomes
First Year
As a result of meeting with a Coordinated Care Advisor, students will:
- Build a sense of belonging
- Identify and create genuine connections with staff, faculty and other students inside and outside the classroom to ensure students are creating a supportive and caring environment for themselves.
- Develop mindful self-awareness
- Reflect on their personal values, academic goals and decision-making processes as it relates to their participation and responsibility to the SDSU community.
- Understand and utilize navigational tools
- Navigate, utilize and differentiate my.SDSU, SDSU Navigate, Transferology and University Catalog to identify and select required course work and connect with their Success Team.
- Differentiate between the different advising tools and their purpose.
- Experience decreased anxiety
- Envision a positive future-self
- Articulate their academic goals, identify their resources and develop their milestones based on their academic and professional goals.
Second Year
As a result of meeting with a Coordinated Care Advisor, students will:
- Transition and apply their vision of a positive future self:
- Feel empowered to implement or progress in their 1st year academic and professional milestones and acknowledge their transition to an upper division status.
- Enhance their engagement in student activities and organizations and co-curricular activities:
- Be active participants in the SDSU and/or larger San Diego community by identifying a community that they have joined or intentionally engaged with.
